Many weapons and shields in Demon's Souls can be upgrade by a Blacksmith, but you will need Souls to pay the Blacksmith and a collection of Ores as well. There are 14 different Ores, which allow for 14 different upgrade paths, but some Ores can only be used on some weapons, some only on shields. There are also three upgrade types. The first type, which can be performed almost immediately by Blacksmith Boldwin is referred to as the basic and will improve the physical damage of a weapon. The second upgrade type is referred to as the advanced type and will have major changes to weapons attributes. With the exception of Clearstones, only Blacksmith Ed can perform the advance upgrades.
The third upgrade type is a Demon's Soul upgrade and comes in two flavors. The first Soul upgrade will convert an existing weapon into a Unique Weapon. This will require a particular weapon and a Demon's Soul. The second Soul upgrade type can only be performed on certain Unique Weapons using Colorless Demon's Souls. Again, only Blacksmith Ed can be perform Soul based upgrades.
A number of the advance and Soul upgrades can not be performed until a weapon has reached a certain basic level.

Basic upgrades
The Hardstones and Sharpstones Ores are used on the basic weapon upgrades. Strength focused weapons can be upgraded with Hardstones, while Dexterity based weapons are upgraded with Sharpstones. Both Blacksmith Boldwin and Blacksmith Ed can perform basic upgrades. There are 10 upgrade steps on the basic upgrade line and a weapon or shield will have the current upgrade step added to the items name. For example, a Claymore +10 is a fully upgraded Claymore.
A fully upgraded Hardstone or Sharpstone weapon will have double the base weapons damage, while upgraded shields will take less of your Stamina with each blocked hit.
All weapons and shields except for the Club, Great Club, Broken Sword and all the Unique Weapons can be upgraded along one of the basic paths.
Upgraded basic weapons can be Enchanted with either spells or items.
Advance upgrades
With the advanced upgrades, attributes are changed that affect the behavior of the weapon or shield. A weapon is not simply improved, it is changed. These changes can be major like adding Fire or Magic damage, the chance to critically injure, to bleed or poison an enemy. A few upgrades will add HP or MP regeneration. Other changes may not appear like a step forward until the weapon is fully upgraded, like having a S stat bonus at full upgrade. A number of the upgrade paths suit the early stages of the game, like Clearstone and Dragonstone. Yet others suit the mid to late stages - depending on how you build your character and how you play.
With the exception of Clearstone upgrades, all advance upgrades are performed by Blacksmith Ed. All advance path upgrades go to a +5 level.

Soul upgrades

The first type of Soul upgrade is performed by Blacksmith Ed by combining a certain Demon's Soul and a particular weapon. This will then create a Unique Weapon. The second type of Soul upgrade is performed on a limited number Unique Weapons with a Colorless Demon's Soul. The supported weapons and shields can be upgraded to +5 with each upgrade costing one Soul. Each upgrade will improve certain aspects of the weapon, some of these upgrades are not very obvious, so check the relevant weapons' page before upgrading.
