I'm not too sure, but once you invade someone's world, he will see a black fog, so he might quit hes game too avoid being killed, wich would explain why the ''session ended''. also if u wanna join or ur being summoned, and the host DIES, it will also lead too the sessions being ended.

Practically what the poster above me mentioned. Though sometimes you can just simply fail to join the session due to connection issues and etc. I remember trying to go co-op/red eye stone against some of my friends that i've met on the forums and sometimes it'd simply just say "failed to join the session" or "session has ended".

Try port forwarding.
Go to PortForward.com (http://portforward.com/english/routers/port_forwarding/routerindex.htm)
Look for your router model
Look for PS3 Demon's Souls
Follow the instructions to forward/open the required ports

I'm no network techie, but portforwarding, having a static IP address, and using some specified DNS numbers I got from Gamefaqs solved my frequent disconnect issues. :)

I'm sorry I was wrong, it was GameSpot. I can't find the link too, but I'll send you the DNS numbers via PSN the next time I play. Just remind me though when we meet in the chatroom or send me a message perhaps. :)

I saved the DNS numbers I got from the GameSpot forums in my phone. Just found out now. :)
Disclaimer: I don't know if these DNS numbers, portforwarding, or static IP address made my connection stable because I changed all of the above at the same time. Use with your own discretion. :)
Primary DNS: 208.67.222.222
Secondary DNS: 208.67.220.220
