Yes just leave one boss to beat otherwise you can upgrade anymore.

When you beat the first playthrough, the game will start over and you will be in your first new game (ng). You'll have all your souls, but you wont be able to use them until you beat the first world boss and return to the nexus. Then just do the same thing as before-talk to maiden, monumental, then maiden again.

yes you keep everything on you, and you bank stays the same with all of your stuff

watch out the difficulty increases by 40% be prepared buy all the grasses,spices, arrows etc before you hit new game+

depends, id get your vitality up pretty high.

I think the average people beat first playthrough is sl 80-100, so you should be fine if you're within that range.
